Form/e-mail script werkt in FireFox niet

Ik gebruik het simplemail.cgi script maar dat werkt niet in FireFox. Het formulier wordt niet verzonden en de "responspage" werkt ook niet.

Wat moet ik veranderen om dit wel te laten werken

Nog 2 andere vragen: in de email die verzonden wordt staan nogal wat extra lege regels. Hoe kan ik die er uit halen.
Hoe kan ik dezelfde email ook naar de afzender sturen die zijn email adres invult in het veld genaamd "email" zodat dit als afzender in de email komt te staan. Helaas blijkt het niet mogelijk hier via autorespond automatisch een email naar de afzender te laten sturen zodat deze ook een kopie van hetgeen ingevuld in het formulier ontvangt.
